Face volume loss: why it happens at your age

Volume loss looks different at 28 than at 58, and the right treatment changes too. Here is how our doctors in Sinsa think about the causes by age — and when they choose filler versus a collagen stimulator.

Why the face loses volume — by age group

The face has several separate fat compartments sitting on bone, held by ligaments and covered by skin. They age at different speeds, which is why volume loss looks different in each decade. This page focuses on causes by age and on choosing a product; for a general overview, see face volume loss in Korea.

20s and 30s

Usually not ageing: rapid weight loss, endurance training, low body fat or simply genetics can hollow the temples, flatten the front cheek or deepen the tear trough. Skin is still elastic, so small, precise amounts restore shape well.

40s

Deep fat pads begin to thin and the cheek fat slides lower. The upper cheek looks flatter while the lower face looks heavier, and hollow under-eyes and folds deepen. Skin quality starts to matter as much as volume.

50s and beyond

Bone around the eyes, cheeks and jaw gradually recedes, removing the base that fat and skin rest on. Combined with thinner skin and laxity, the face can look gaunt and saggy at the same time. Wide, gradual support usually works better than large amounts of filler in a few spots.

Choosing filler vs collagen stimulators

Both restore volume, but they work in different ways. The doctor considers your age group, skin thickness, how quickly you want a result and how long you will be in Seoul.

Option How it works Result timeline From
HA filler Hyaluronic acid gel adds volume directly; dissolvable Immediate; lasts ~6–18 months ₩180,000

Radiesse Calcium hydroxylapatite: instant support plus collagen stimulation Immediate, then builds ₩950,000
Juvelook Volume PDLLA stimulates collagen gradually Builds over 1–3 months ₩400,000
Sculptra PLLA stimulates collagen over a wide area Builds over 2–3 months; often lasts up to ~2 years ₩500,000

  • Filler often suits younger patients with a specific hollow, anyone who wants an immediate result before an event, and areas needing precise shape such as the chin or tear trough.
  • Collagen stimulators such as Radiesse often suit people in their 40s and older with wider, gradual loss and thinning skin, who can return for 2–3 sessions and prefer a slow, natural change.
  • Many plans combine both: a little filler for structure and a stimulator for overall quality.

What changes when volume is restored

Before · hollow cheeks After · softer contours

With filler, the change is visible right away and settles as swelling fades over a week or two. With stimulators such as Juvelook Volume or Sculptra, the face looks slightly fuller on the day, flattens again, then gradually fills in over 2–3 months as collagen forms. The aim at any age is a rested, natural shape — not obvious fullness.

How a volume restoration visit works

  1. Age-based assessment The doctor looks at fat, bone, skin and ligaments to decide where volume is truly missing.
  2. Product choice Filler, collagen stimulator or a combination, with a clear plan for the number of sessions.
  3. Injection After about 30 minutes of numbing, injection takes 20–45 minutes with a needle or cannula.
  4. Aftercare Cooling and instructions — including the 5-5-5 massage routine after Sculptra, if used.

Short trip? Filler gives an immediate result with a check before you fly. Collagen stimulators need repeat sessions 4–6 weeks apart, so they suit patients who visit Seoul regularly or live here.

Downtime, side effects and safety

Volume injections are medical procedures in areas rich in blood vessels. What to expect:

  • Common: swelling, tenderness and bruising for 3–7 days; with stimulators, the initial fullness from the liquid fades within days before collagen builds.
  • Less common: lumps or unevenness, swelling under the eyes lasting several weeks, small nodules after collagen stimulators.
  • Rare: infection, delayed inflammatory reactions, or vascular occlusion (a blocked blood vessel), which needs urgent treatment; HA filler can be dissolved (see filler prices), stimulators cannot.

Tell the doctor first if

you are pregnant or breastfeeding, have an autoimmune disease, take blood thinners, have had previous filler or non-dissolvable fillers in the area, have a history of keloids, or have any active infection. Avoid saunas, alcohol and face-down massage for about a week.

Volume loss FAQ

How much does face volume restoration cost in Gangnam?

At Bailor Clinic in Sinsa, filler starts from ₩180,000 (chin 1–2cc), Juvelook Volume 4cc is ₩400,000, Sculptra starts from ₩500,000 and Radiesse from ₩950,000, VAT excluded. See Sculptra prices.

Is filler or Sculptra better for a thin face?

It depends on age and skin. Filler gives an immediate, targeted result; Sculptra builds gradually over a wider area and may last longer. Many people in their 40s and 50s benefit from stimulators, sometimes with a little filler for structure.

Why does my face look hollow in my 20s or 30s?

Most often weight loss, intense exercise, low body fat or genetics, rather than ageing. Small, precise filler amounts usually restore shape well at this age.

How many sessions do collagen stimulators need?

Usually 2–3 sessions about 4–6 weeks apart, with results building over 2–3 months.

Can filler be reversed?

Hyaluronic acid filler can be dissolved with an enzyme if needed. Collagen stimulators such as Sculptra and Radiesse cannot be dissolved in the same way, which is why careful planning matters.
