What is acne scar filler?
Deep inflamed acne can leave scars that are not just a surface mark but a small pocket of lost tissue held down by fibrous bands. These are the soft, wave-like rolling scars and wider boxcar scars that look worse in side lighting. Resurfacing the surface helps texture, but it does not cut those bands, so the scar can stay anchored.
Scar filler tackles the problem from underneath in two steps:
- Subcision — a fine needle or cannula is passed under the scar to release the tethering bands.
- Support — a collagen-stimulating filler, typically Juvelook, is placed in the released space so the scar does not re-attach and new collagen can fill it in over time.
It is less useful for narrow, deep ice pick scars, which usually need targeted methods instead. That is why the doctor maps each scar type before choosing a plan.

Before you start
Active breakouts should be calm first, because injecting through inflamed skin raises the risk of infection. If you still get new spots, the doctor may start with CAPRI laser or acne care and begin scar work once the skin is stable. For a broader view of all scar options, see our acne scar treatment page.
What changes after scar filler
Some lift is visible once swelling settles, but most improvement comes as collagen builds over 1–3 months after each session. Scars usually become shallower and less shadowed rather than disappearing completely; remaining surface texture can be refined with fractional laser.
How a scar filler session works
- Scar mapping Under angled light, the doctor marks rolling, boxcar and ice pick scars and agrees the plan with you.
- Numbing Numbing cream for 30–40 minutes; local anaesthetic can be added where subcision is done.
- Subcision & filler Tethered scars are released and Juvelook placed beneath them, then any laser or RF step follows.
- Calming care Cooling and a calming mask; you leave with aftercare instructions and WhatsApp access.
Because bruising can last up to a week, international patients usually have their first session early in the trip. Sessions are spaced about four weeks apart, so many visitors plan the full course across return visits.
Downtime, side effects and who should wait
Subcision and filler are more involved than a facial, and honest preparation matters.
- Common: swelling, bruising and tenderness for 3–7 days; small bumps where filler was placed; redness after laser or RF steps.
- Less common: firm nodules, uneven correction or temporary darkening of the skin (post-inflammatory hyperpigmentation), especially in darker skin types.
- Rare: infection, allergic reaction or blood-vessel compromise; contact us at once about increasing pain, blanching or colour change.
Tell the doctor first if
You are pregnant or breastfeeding, are taking or recently took isotretinoin, use blood thinners, have a history of keloids or cold sores, or have active acne in the area. Pregnant patients should postpone treatment.
Aftercare
Avoid makeup for 24 hours, skip saunas, alcohol and hard exercise for about a week, do not massage the area unless instructed, and use SPF daily to reduce marks.

Scar filler FAQ
How much does acne scar filler cost in Gangnam?
At Bailor Clinic in Sinsa, scar filler plans range from ₩400,000 (BASIC) to ₩1,200,000 (SIGNATURE) per session, with packages of ₩1,150,000 (4 sessions) and ₩1,850,000 (Juvelook, 5 sessions), VAT excluded. See the acne scar price list.
Which acne scars respond best?
Rolling scars and soft boxcar scars respond best, because they are held down by bands that subcision can release. Narrow ice pick scars usually need other methods.
How many sessions will I need?
Most people need 3–5 sessions about four weeks apart. Deeper or widespread scarring may need more.
Is scar filler painful?
Numbing cream and local anaesthetic are used, so most patients feel pressure and some stinging. Sensitivity varies by area.
Can I have scar filler while I still get acne?
Active acne should be controlled first to reduce infection risk. The doctor may start with acne treatment and add scar filler once the skin is calm.
